RiskNote
5x5 risk matrix

A risk matrix that speaks for itself

A clickable 5x5 matrix with likelihood on the x-axis and consequence on the y-axis. Every risk is auto-coloured by its score. Export to PDF or screenshot in one click.

The standard, minus the spreadsheet

The 5x5 matrix is the most widespread tool in the ISO 31000 world for a reason: it's visually immediate. A red cell in the top-right is critical. A green cell in the bottom-left is deprioritised. Boards understand the picture without explanation.

The problem is that most organisations build the matrix in PowerPoint or Excel. That means manual updates, inconsistent colours, and no way to click into a risk to see details. RiskNote makes the matrix live.

What you get in RiskNote's matrix

  • Automatic score calculation

Likelihood (1–5) × Consequence (1–5) = Severity. Colours (green, yellow, orange, red) are assigned automatically based on thresholds aligned with ISO 31000 guidance.

  • Clickable cells with drilldown

Click a cell and see every risk at that position. Move a risk by updating its likelihood or consequence — the matrix reflows live.

  • Heat map across the register

See concentration at a glance. Too many risks in the top-right? Too many sitting in the bottom-left? The picture tells you where attention is needed.

  • PDF export preserves the layout

The matrix is rendered as part of the exported report. Your board sees the same colours and layout you see in the product — no Excel-import drift.

  • Nordic-professional palette

Discreet and professional. Not Excel-neon, not PowerPoint-pastel. Works in both light and dark themes, on screen and print.

How to build a matrix

  • 1. Add risks (manually or via AI)

Each risk gets a title, description, likelihood, and consequence. Add them manually or let the AI propose risks as a starting point.

  • 2. Adjust likelihood and consequence

Click a risk to change its values. The matrix updates immediately — you see in real time how the change affects the heat map.

  • 3. Export or present

When the matrix is ready, click ”Export PDF”. You get a report with the matrix on the cover, the register on the next page, and an AI disclosure in the footer.

Included in every plan

The 5x5 matrix is a core feature included in all plans, including the 7-day free trial. Unlimited at every tier.

Risk matrix FAQ

Do you support matrix sizes other than 5x5?

Not right now. 5x5 is the most widespread standard and gives enough granularity without being overwhelming. If you have a specific requirement (e.g. 3x3 or 4x4), get in touch — we're evaluating more flexibility for a future version.

Can I customise the colour thresholds?

Defaults follow ISO 31000 guidance and work for most organisations. Threshold customisation is planned for V1.2.

Is the matrix exported in vector format?

Yes. The PDF export uses vector graphics, so the matrix stays crisp at any zoom level or print size — including A3 presentations.
